Medical Leave of Absence

Requesting a Medical Leave of Absence

A student wishing to take a medical leave of absence must first discuss this request with a representative of the Office of Student Assistance. They can be reached by calling 315-443-4357.

The Office of Student Assistance will notify Health Services of the student’s request to take a medical leave of absence and a form that will include the information below from Health Services will be e-mailed to the students syr.edu account.

In order to take a medical leave of absence from Syracuse University, the completed form along with documentation* from your licensed health care provider must be submitted to Health Services and approved by the Medical Director or appropriate designee.

The completed form your licensed health care provider must be received within thirty (30) days of the date of your departure and should include the following information:

  • A medical diagnosis.
  • The date that the medical diagnosis made it necessary to take a medical leave.
  • The length of time that the provider has been treating the medical condition.
  • The anticipated recovery time for the diagnosis.
  • Is the condition permanent or cyclical.
  • Describe the impact on academic performance if the medical leave is not granted.
  • What date was the last scheduled visit.
  • In the providers medical opinion, is a medical leave of absence required and if so, explain in detail.
  • Confirmation of diagnosis and its serverity must be provided. Confirmation can include relevant progress notes, diagnostic tests and/or treatment.

Once Health Services has received the completed form, you will be notified in writing as to whether your medical leave of absence has been approved, denied or if additonal documentation is needed.

Readmission to the University

Should you wish to return to Syracuse University after taking a medical leave of absence, Health Services will need additional documentation* from your licensed health care provider. Your return to Syracuse University must also be approved by the Medical Director or appropriate designee.

The documentation from your licensed health care provider should be a summary letter which includes the following information:

  • A statement that you are cleared to return to Syracuse University without any restrictions.
  • Provide a date that you are able to return to classes.
  • The summary letter must be on office letterhead and must also be signed by your licensed health care provider.

Once Health Services has received the documentation and your return from a medical leave of absence has been approved, you will be contacted by the Office of Student Assistance.

Readmission Deadlines

Please take note of the deadlines below.  Failure to meet a deadline might result in your return to Syracuse University being postponed until the following semester. Additionally, it is your responsibility to contact your home school/college for specific academic requirements regarding your re-enrollment.

Readmission for FALL semester:

Supporting documents to Health Services by August 1st

Readmission for SPRING semester:

Supporting documents to Health Services by December 15th

Readmission for SUMMER semester:

Supporting documents to Health Services by April 15th
