Health Services Requirements for NEW Fall 2018 Students

Due July 1

Under guidelines established by New York State Public Health Law, Syracuse University policy requires that all students provide:
  • Documentation of two (2) doses of measles, mumps, and rubella (MMR) or proof of immunity.
  • Documentation of one (1) dose of meningococcal meningitis ACWY vaccine within the preceding five years and after the age of 16 or a completed waiver form declining the vaccination.
Your Health History and Immunization Forms* are due by July 1. Accessing your housing assignment or class registration cannot be done until you submit your vaccination records, so be sure to submit your records using the Health Services Patient Portal. To log in, use your SU Net ID and password.

Health Services is available to assist you in obtaining the appropriate vaccinations if you have not be vaccinated or do not have documentation of your vaccinations. *Note: The mandatory immunization requirement form (on the patient portal) MUST be completed and received by Health Services prior to your arrival on campus. If these documents are not received, students will risk strict administrative consequences including the inability to register for classes and/or suspension of ID card services.
