Submitting Immunization Data

In accordance with New York State Public Health Law, Health Services requires that all students provide:

  • Documentation of two (2) doses of measles, mumps, and rubella (MMR) or proof of immunity.
  • Documentation of one (1) dose of meningococcal meningitis ACWY vaccine within the preceding five years and after the age of 16 or a completed waiver form declining the vaccination.

Students must submit immunization data to Health Services using the Patient Portal. The process involves two parts:

  • Entering the dates when you received required immunizations
  • Uploading proof of immunizations

To accomplish this, follow the steps in the sections below.

Entering Immunization Dates

  1. Visit and login in using your NetID and password.
  2. On the Welcome page, click on the Immunizations menu and select the Enter Dates menu item.
  3. On the resulting web page (see below), enter appropriate dates for each of the required immunizations.
  4. If you have alternates to the required immunizations, scroll to the bottom of the page and enter titers or individual vaccines.
  5. Click the Submit button.

Uploading Proof of Immunizations

Before you start: Please have a completed immunization form filled out by your healthcare provider OR have on hand copies of official immunization records (i.e. health care provider, school, or miliary).

  1. Scan and save your immunization form or documents individually to your computer or take a picture with your smart phone. If you are uploading multiple documents, you will need to scan/take a picture and save each document as a separate file. Please Note: Images must be one of the following formats: .gif, .png, .tiff, .jpg, .jpeg. Documents must be either .txt or .pdf files. Also, file must be smaller than 4 MB. Scan in black and white, or at a setting of 150 DPI to achieve a smaller file.
  2. Visit and login in using your NetID and password.
  3. On the Welcome page, click on the Uploads button towards the bottom of the page.
  4. On the resulting web page, there is a list of documents that can be uploaded. Choose Immunizations Form from the "Choose document you are uploading" drop down menu. 
  5. Click on Select File, and locate and select the file on your computer or smart phone.
  6. After selecting the file(s) you wish to upload, click on the Upload button.
